The Federal Motor Carrier Safety Administration (FMCSA) has issued a temporary exemption allowing interstate CDL and CLP holders to use a paper Medical Examiner’s Certificate as proof of certification for up to 60 days after the examination date. That exemption ends October 11, 2026.

After the transition, electronic medical certification information will be transmitted through the FMCSA National Registry and state driver records. Your paper card may still be useful for your records, but your medical certification must be properly reported and current in the applicable electronic system.

What the October 11 Change Means

A DOT medical card is formally called a Medical Examiner’s Certificate, or MCSA-5876. It confirms that you completed a DOT physical with a medical examiner listed in the FMCSA National Registry.

FMCSA states that the temporary paper-card exemption is in effect from April 11, 2026, through October 11, 2026. FMCSA also recommends that certified medical examiners continue issuing paper certificates while examination results are submitted electronically during the transition.

Louisiana has also announced that, beginning September 15, 2026, medical providers will electronically upload the existence of a DOT medical certificate to FMCSA. Allow time for processing. Louisiana advises that medical information may take up to 10 business days after submission to appear in the database.
